This form must be completed by a parent/guardian and returned to the school nurse before you have a sports physical completed by the school physician or your family physician.

This form is to be filled out by a parent/guardian if you have had a physical and are planning to join a second or third team during the school year.

If you decide that, you want your sports physical to be done by your family doctor, this is the form that your doctor must complete and return to the school nurse.
