School-Based Health Center Parent Survey

The Whitney Young School-Based Health Center is asking for feedback from parents and guardians of students in the Watervliet City School District.

This year, Whitney Young Health opened a school-based health center (SBHC) at Watervliet Jr.-Sr. High School that provides primary and preventative medical services to students from prekindergarten through grade 12 whose parent have enrolled them in the program. Feedback from the survey will help shape future educational programs and services for the Watervliet community. The survey is voluntary and confidential.

Survey participants will be entered into the drawing for gift cards by filling out the last question on the survey that asks for an email or a phone number. Participants will not be contacted for any reason unless they have won the raffle.

Please complete one of the two surveys (see links below) based on which school your child/children attend. All district parents are encouraged to take the survey whether or not your child is enrolled in the SBHC program.
